Job Description:

We are seeking a strategic and experienced System Director of Case Management to lead and oversee case management operations across our healthcare system. This leadership role is responsible for ensuring effective care coordination, discharge planning, and resource management to support optimal patient outcomes and regulatory compliance. The ideal candidate brings strong clinical insight, leadership expertise, and a passion for system-level healthcare improvement.

Key Responsibilities:

Lead and manage case management programs across multiple facilities or locations
Develop and implement system-wide strategies for care coordination, utilization review, and discharge planning
Collaborate with hospital leadership, physicians, and interdisciplinary teams to ensure patient-centered care
Monitor performance metrics and drive improvements in length of stay, readmission rates, and care transitions
Ensure compliance with CMS regulations, payer requirements, and accreditation standards
Mentor and support case management leaders and staff across the organization
Oversee resource allocation, staffing models, and budget management
Lead initiatives related to population health, social determinants of health, and value-based care

Qualifications:

Current RN license (required); CCM or ACM certification strongly preferred
Bachelor’s degree in Nursing required; Master’s degree in Nursing, Healthcare Administration, or related field preferred
7+ years of progressive experience in case management, care coordination, or utilization review
3+ years in a healthcare leadership or system-level management role
Deep knowledge of clinical workflows, discharge planning, and payer practices
Proven ability to lead change, mentor teams, and manage multi-site operations
Exceptional communication, organizational, and analytical skills
