Registered Nurse (RN) – Behavioral Health Care Manager

🩺 Job Summary:

We are seeking a compassionate and experienced Registered Nurse (RN) to serve as a Behavioral Health Care Manager. In this role, you will coordinate and manage care for patients with behavioral and mental health conditions. The RN Care Manager works collaboratively with physicians, therapists, and community resources to ensure integrated, patient-centered care and optimal outcomes.

🔹 Key Responsibilities:

Assess behavioral health needs and develop individualized care plans.
Provide care coordination for patients with mental health and substance use disorders.
Monitor patient progress, evaluate treatment outcomes, and adjust care plans accordingly.
Collaborate with multidisciplinary teams including psychiatrists, therapists, and case managers.
Educate patients and families on mental health conditions, treatment options, and self-management.
Facilitate referrals to appropriate community and healthcare resources.
Maintain accurate documentation and ensure compliance with clinical standards and regulations.

🎓 Qualifications:

Valid Registered Nurse (RN) license.
Minimum of 2 years of behavioral or mental health experience preferred.
Strong understanding of psychiatric disorders, crisis management, and care coordination.
Excellent communication, problem-solving, and organizational skills.
Ability to work independently and within an interdisciplinary team.
