Registered Nurse (RN) – Case Manager (OB/GYN)

🩺 Job Summary:

As an OB/GYN RN Case Manager, you are the bridge between clinical expertise and emotional understanding — guiding patients through the most transformative moments of their lives. You’ll coordinate individualized care plans for women through pregnancy, postpartum, gynecological procedures, and reproductive health concerns, ensuring continuity of care across multidisciplinary teams.

🔹 Key Responsibilities:

Perform comprehensive patient assessments for obstetric and gynecological care.
Develop and implement evidence-based, individualized care plans.
Coordinate care among physicians, midwives, social workers, and external providers.
Monitor high-risk pregnancies and manage follow-up for chronic women’s health conditions.
Provide patient education on prenatal, postpartum, and reproductive health topics.
Ensure seamless discharge planning and follow-up for optimal outcomes.
Maintain accurate clinical documentation and compliance with quality standards.

🎓 Qualifications:

Active Registered Nurse (RN) license.
BSN preferred; certification in Case Management or Maternal Health a plus.
2+ years of OB/GYN, maternity, or women’s health nursing experience required.
Deep understanding of maternal-fetal medicine, women’s health, and emotional support strategies.
Strong communication, empathy, and coordination skills.
