Job Title: LVN/LPN for Long Term Acute Care (LTAC) in Claremont, New Hampshire
Job Description: Are you an experienced LVN/LPN looking for an exciting opportunity to work in a Long Term Care facility? Our nursing team in Claremont, NH is seeking a temporary LVN/LPN to join us for day shifts. As a temporary LVN/LPN, you will be responsible for providing compassionate care to our residents, administering medications, and assisting with daily living activities.
Responsibilities:
Administer medications as prescribed by physicians
Provide direct patient care and assistance with daily living activities
Monitor patient health and report any changes to the nursing supervisor
Assist with the development and implementation of care plans
Maintain accurate medical records
Requirements:
Current LVN/LPN license in the state of New Hampshire
Minimum of 1 year of experience in Long Term Care or related field
Ability to work independently and as part of a team
Strong communication and interpersonal skills
