Job Title: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) Long Term Care (LTC)
Job Summary:
The Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) in Long Term Care (LTC) settings provides nursing care to patients in long-term care facilities, such as nursing homes, rehabilitation centers, and assisted living facilities. They work under the supervision of registered nurses or physicians, and are responsible for assisting with patient assessments, medication administration, wound care, and patient education.
Responsibilities and Duties:
1. Administer medications and treatments to patients as prescribed by healthcare providers.
2. Monitor and assess patient conditions, including vital signs, and report any changes to the healthcare team.
3. Assist patients with activities of daily living, such as bathing, dressing, and eating.
4. Provide wound care and assist with dressing changes as needed.
5. Educate patients and their families on proper healthcare practices and medication management.
6. Document patient information and care provided accurately and in a timely manner.
7. Collaborate with other healthcare team members to ensure comprehensive and coordinated patient care.
8. Maintain a clean and safe environment for patients.
9. Adhere to all healthcare regulations and policies.
Qualifications and Skills:
1. Must be a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of practice.
2. Previous experience in long-term care or geriatric nursing is preferred.
3. Strong communication and interpersonal skills.
4. Ability to work effectively in a team environment.
5. Basic computer skills for documentation purposes.
6. Ability to prioritize and multitask in a fast-paced environment.
7. Compassionate and patient-centered care approach.
8. Current BLS certification.
