Job title: Registered Nurse (RN) - Home Health
Job Summary:
The Registered Nurse (RN) - Home Health will be responsible for providing quality nursing care to patients in the comfort of their own homes. This individual will assess patients' health needs, develop and implement care plans, and provide education to patients and their families. The RN will work closely with other healthcare professionals to ensure the best possible outcomes for patients.
Responsibilities and Duties:
- Conduct initial assessments of patients' health status
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed
- Monitor patients' progress and update care plans as needed
- Provide education to patients and their families on managing their health conditions
- Collaborate with other healthcare professionals to coordinate care
- Document all patient care activities accurately and timely
- Maintain confidentiality of patient information
- Comply with all regulatory requirements and standards of practice
Qualifications and Skills:
- Valid RN license in the state of practice
- Minimum of 2 years of nursing experience, with previous home health experience preferred
- Strong assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of electronic medical records and basic computer skills
- Current CPR certification
- Reliable transportation and valid driver's license
