Job Title: Social Worker
Job Summary:
A Social Worker is responsible for providing support and assistance to individuals and families in need. They work to improve the well-being of clients by assessing their needs, developing treatment plans, and connecting them to resources and services in the community.
Responsibilities and Duties:
1. Conduct assessments to determine the needs of clients.
2. Develop individualized treatment plans and goals.
3. Provide counseling and support to clients.

5. Advocate for clients and ensure they receive the services they need.
6. Monitor client progress and adjust treatment plans as necessary.
7. Maintain accurate and up-to-date client records.
8. Collaborate with other professionals, such as doctors, therapists, and case managers.
9. Participate in team meetings and case conferences.
10. Provide crisis intervention and support in emergency situations.
Qualifications and Skills:
1. Bachelor's or Master's degree in Social Work or related field.
2. State licensure or certification as a Social Worker.
3. Experience working with diverse populations and individuals with complex needs.
4. Strong communication and interpersonal skills.
5. Ability to work independently and as part of a team.
6. Knowledge of community resources and social service agencies.
7. Excellent problem-solving and decision-making abilities.
8. Compassion and empathy for clients in need.
9. Cultural competency and sensitivity to differences in backgrounds and experiences.
