Job Title: Registered Nurse (RN) - Home Health
Job Summary:
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in the comfort of their own homes, while promoting their independence and overall well-being. As an RN in Home Health, you will play a vital role in coordinating and delivering personalized care plans, monitoring patient progress, and educating patients and their families on self-care techniques.
Responsibilities and Duties:
- Conduct initial assessments of patients to determine their individual needs and develop comprehensive care plans
- Administer medications, perform wound care, and provide other nursing interventions as prescribed by healthcare providers
- Collaborate with interdisciplinary team members to ensure holistic care and continuity of services
- Educate patients and their families on disease management, healthy lifestyle choices, and medication management
- Monitor patient progress and update care plans as needed
- Provide emotional support and comfort to patients and families during challenging times
- Document all patient interactions, interventions, and outcomes accurately and timely
- Adhere to all regulations and standards of practice in Home Health nursing
Qualifications and Skills:
- Valid RN license in the state of practice
- Minimum of 1 year of nursing experience, preferably in Home Health or a similar setting
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work independently and as part of a team
- Knowledge of Medicare and Medicaid regulations for Home Health
- CPR certification
- Reliable transportation
- Ability to lift and move patients as needed
