Job Title: Registered Nurse (RN) - Hospice
Job Summary:
As a Registered Nurse (RN) specializing in hospice care, you will be responsible for providing compassionate and holistic nursing care to patients at the end of life. You will work closely with patients, families, and interdisciplinary teams to ensure comfort, dignity, and quality of life during this sensitive time.
Responsibilities and Duties:
1. Conduct comprehensive assessments of patients' physical, psychological, and spiritual needs
2. Develop and implement individualized care plans in collaboration with the hospice team
3. Administer medications and treatments in accordance with physician orders
4. Provide emotional support and education to patients and families
5. Coordinate and communicate with other healthcare professionals involved in the patient's care
6. Monitor and evaluate patient responses to care and adjust plans as needed
7. Participate in end-of-life discussions and advance care planning
8. Maintain accurate and timely documentation of all patient interactions and care provided
Qualifications and Skills:
1. Current RN license in the state of practice
2. Minimum of 2 years of clinical experience in hospice or palliative care
3. BSN or MSN degree preferred
4. Certification in hospice and palliative care (CHPN) preferred
5. Strong communication and interpersonal skills
6. Compassionate and empathetic approach to patient care
7. Ability to work effectively in a team environment
8. Knowledge of end-of-life care principles and best practices
