Job Title: Pharmacist Inpatient
Job Summary:
The Pharmacist Inpatient is responsible for providing pharmaceutical care to patients admitted to a hospital or healthcare facility. They work closely with healthcare providers to ensure safe and effective medication use for inpatients.
Responsibilities and Duties:
1. Review patient medication orders for accuracy and appropriateness.
2. Prepare and dispense medications in a timely manner.
3. Monitor patient response to drug therapy and adjust medications as needed.
4. Provide drug information to healthcare providers and patients.
5. Collaborate with other healthcare team members to optimize patient outcomes.
6. Maintain accurate medication records and documentation.
7. Participate in hospital committees and initiatives related to medication safety and quality improvement.
Qualifications and Skills:
1. Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.
2. Active pharmacist license in the state of practice.
3. Prior experience in an inpatient pharmacy setting preferred.
4. Strong clinical pharmacy skills and knowledge of drug therapy.
5. Excellent communication and interpersonal skills.
6. Ability to work in a fast-paced environment and prioritize tasks effectively.
7. Attention to detail and accuracy in medication dispensing.
