Job Title: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) Long Term Care (LTC)
Job Summary:
As a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in Long Term Care (LTC) facilities, you will be responsible for providing direct patient care and support to residents in a long-term care setting. You will work under the supervision of registered nurses (RNs) to ensure the overall health and wellbeing of residents.
Responsibilities and Duties:
- Administer medications and treatments as prescribed by healthcare providers
- Monitor residents' vital signs and assess their overall health status
- Assist with daily living activities, such as bathing, dressing, and eating
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Provide emotional support and comfort to residents and their families
- Document all care provided and communicate any changes in residents' conditions to the healthcare team
- Adhere to all state and federal regulations governing long-term care facilities
- Maintain a clean and safe environment for residents
Qualifications and Skills:
- Graduation from an accredited LVN or LPN program
- Current and valid license to practice as an LVN or LPN in the state of employment
- CPR certification
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
- Attention to detail and strong critical thinking skills
