Job Title: Pharmacist Inpatient
Job Summary:
The Pharmacist Inpatient plays a crucial role in providing medication management services to patients within a hospital setting. They work closely with physicians, nurses, and other healthcare professionals to ensure safe and effective medication use for inpatients.
Responsibilities and Duties:
1. Reviewing medication orders for accuracy and appropriateness.
2. Compounding and dispensing medications in accordance with physician orders.
3. Monitoring patient medication therapy for adverse reactions and interactions.
4. Providing drug information to healthcare professionals and patients.
5. Collaborating with healthcare team to develop treatment plans.
6. Participating in patient education on medication use and side effects.
7. Overseeing pharmacy technicians in medication preparation and distribution.
8. Ensuring compliance with all pharmacy regulations and standards.
9. Contributing to quality improvement initiatives within the pharmacy department.
10. Maintaining accurate patient medication records.
Qualifications and Skills:
1. Doctor of Pharmacy (PharmD) degree from an accredited pharmacy program.
2. Active pharmacy license in the state of practice.
3. Completion of a pharmacy residency program (preferred).
4. Strong knowledge of pharmacotherapeutics and medication management.
5. Excellent communication and interpersonal skills.
6. Ability to work effectively in a fast-paced healthcare environment.
7. Attention to detail and accuracy in medication dispensing.
8. Proficiency in using pharmacy software and technology.
