Job Title: Registered Nurse (RN) MDS Coordinator
Job Summary:
As a Registered Nurse (RN) MDS Coordinator, you will be responsible for coordinating and completing Minimum Data Set (MDS) assessments for residents in a skilled nursing facility. You will work closely with interdisciplinary teams to ensure accurate and timely assessments are conducted in accordance with regulations and guidelines.
Responsibilities and Duties:
1. Conduct MDS assessments for residents in compliance with state and federal regulations.
2. Coordinate care plans with interdisciplinary teams to ensure individualized care for residents.
3. Monitor and evaluate resident care plans to ensure quality of care and compliance with regulations.
4. Communicate with healthcare providers, staff, and families to ensure continuity of care.
5. Maintain accurate and up-to-date records of assessments and care plans.
6. Participate in quality improvement initiatives to enhance resident care and outcomes.
7. Provide education and training to staff on MDS assessments and documentation requirements.
Qualifications and Skills:
1. Valid RN license and clinical experience in a skilled nursing facility.
2. Certification in MDS assessment preferred.
3. Strong knowledge of Medicare and Medicaid regulations.
4. Excellent communication and interpersonal skills.
5. Ability to work effectively in a fast-paced and collaborative environment.
6. Attention to detail and strong organizational skills.
7. Proficiency in computer systems and electronic health records.
