Job Title: Registered Nurse (RN) - Case Manager
Job Summary:
We are seeking a compassionate and skilled Registered Nurse (RN) to join our team as a Case Manager. In this role, you will be responsible for coordinating patient care, developing care plans, and ensuring the well-being of our patients throughout their healthcare journey.
Responsibilities and Duties:
1. Assessing and evaluating patients' medical conditions
2. Developing individualized care plans in collaboration with healthcare providers
3. Coordinating and managing patient care to ensure quality and continuity of services
4. Educating patients and their families on treatment options and health management
5. Collaborating with interdisciplinary team members to ensure comprehensive care
6. Monitoring patient progress and adjusting care plans as needed
7. Advocating for patients' needs and rights within the healthcare system
8. Documenting patient information and care plans accurately and timely
9. Participating in case conferences and multidisciplinary team meetings
10. Maintaining current knowledge of healthcare regulations and best practices
Qualifications and Skills:
1. Current RN license in good standing
2. Previous experience in case management or care coordination preferred
3. Strong communication and interpersonal skills
4. Ability to work independently and as part of a team
5. Excellent organizational and time management skills
6. Knowledge of healthcare regulations and best practices
7. Ability to prioritize and multitask in a fast-paced environment
8. Critical thinking and problem-solving abilities
