Job Summary:
The Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) in Long Term Care (LTC) provides nursing care to residents in long-term care facilities. They are responsible for assessing patient needs, developing care plans, administering medication, and providing ongoing support to residents and their families.
Responsibilities and Duties:
1. Assessing and documenting patient needs and conditions
2. Developing and implementing individualized care plans
3. Administering medications and treatments as prescribed by the healthcare provider
4. Monitoring and reporting changes in patient status
5. Providing emotional support and education to residents and their families
6. Collaborating with other healthcare professionals to ensure comprehensive care
7. Maintaining accurate and up-to-date patient records
8. Ensuring compliance with all regulatory standards and policies
9. Adhering to infection control protocols
10. Participating in staff meetings and continuing education activities
Qualifications and Skills:
1. Graduation from an accredited vocational nursing program
2. Current LVN/LPN license in the state of practice
3. Experience working in long-term care facilities preferred
4. Strong communication and interpersonal skills
5. Ability to work effectively as part of a multidisciplinary team
6. Compassionate and patient-centered approach to care
7. Knowledge of nursing principles and practices
8. Ability to prioritize and manage multiple tasks
9. CPR certification
10. Ability to lift and move patients as needed
