Job Title: Registered Nurse (RN) Home Health
Job Summary:
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. As an RN in Home Health, you will be responsible for providing high-quality medical care to patients in the comfort of their own homes. You will work closely with patients and their families to assess their needs, develop care plans, and administer treatments to promote healing and independence.
Responsibilities and Duties:
- Conduct initial assessments and develop individualized care plans for patients
- Provide skilled nursing care, including wound care, medication management, and monitoring vital signs
- Educate patients and their families on managing their health conditions and following treatment plans
- Coordinate with healthcare providers and community resources to ensure holistic care for patients
- Document patient progress and communicate updates to interdisciplinary team members
- Provide emotional support and guidance to patients and their families
Qualifications and Skills:
- Valid RN license in the state of practice
- Minimum of 2 years of clinical nursing experience, preferably in Home Health or a similar setting
- Strong assessment, communication, and organizational skills
- Ability to work independently and as part of a team
- Knowledge of Medicare regulations and documentation requirements
- CPR certification
