📝 Job Description:

We are seeking a compassionate and self-motivated Registered Nurse (RN) to provide skilled nursing care to patients in the comfort of their own homes. As a Home Health RN, you will play a critical role in promoting recovery, patient independence, and overall wellness through individualized care plans and one-on-one support.

✅ Key Responsibilities:

Deliver in-home nursing care including assessments, wound care, IV therapy, medication management, and chronic disease monitoring.
Perform comprehensive initial and follow-up assessments of patients' physical and psychosocial status.
Develop, implement, and regularly update individualized care plans in collaboration with physicians, therapists, and family members.
Educate patients and caregivers on health conditions, medications, and self-care strategies.
Monitor patient progress and adjust care plans as needed.
Ensure accurate and timely documentation using EMR systems.
Comply with all home health regulations, agency policies, and infection control standards.
Travel between patient homes within the assigned territory (mileage reimbursement provided).

🎓 Qualifications:

Active RN license in the practicing state.
Minimum 1 year of clinical nursing experience (home health, med-surg, or acute care preferred).
Valid driver’s license and reliable transportation.
CPR/BLS certification (required).
Strong clinical skills, time management, and ability to work independently.

🌟 Preferred Experience:

Familiarity with OASIS documentation
Experience with EMR systems (e.g., Homecare Homebase, WellSky, etc.)
Knowledge of Medicare/Medicaid guidelines for home health services
