🏡 Registered Nurse (RN) – Home Health | Job Description

Position Overview:

We are seeking a compassionate and self-motivated Registered Nurse (RN) to join our Home Health team. In this role, you will provide direct nursing care to patients in their homes, promote healing, and help maintain or improve their quality of life. The ideal candidate thrives in an autonomous environment and is committed to personalized, patient-centered care.

Key Responsibilities:

• Deliver high-quality nursing care to patients in their homes according to individualized care plans

• Conduct assessments and monitor changes in patient health, reporting critical findings to the physician

• Administer medications, wound care, injections, and IV therapy as prescribed

• Educate patients and families about disease management, medications, and safety

• Document all care activities accurately in the electronic medical record system

• Coordinate care with other healthcare professionals (e.g., therapists, social workers, physicians)

• Ensure compliance with all state, federal, and organizational policies

• Promote a safe, clean, and comfortable environment for the patient

• Travel to patient homes within the assigned geographic area

Qualifications:

• Active and unencumbered Registered Nurse (RN) license in the state of practice

• 1+ years of clinical nursing experience (home health or hospice experience preferred)

• BLS certification required

• Excellent communication, assessment, and critical thinking skills

• Comfortable working independently and managing a flexible schedule

• Reliable transportation and a valid driver's license

Full-Time / Part-Time | Day Shifts | Weekend Rotation May Apply
