Registered Nurse (RN) – Case Manager / Discharge Planner
We are seeking a compassionate and organized Registered Nurse (RN) to serve as a Case Manager / Discharge Planner. The RN will coordinate patient care, manage transitions from hospital to home or other facilities, and ensure continuity of care while supporting patients and families throughout the discharge process.
Key Responsibilities:
Assess patient needs and develop individualized discharge plans.
Coordinate with physicians, nurses, social workers, and insurance providers.
Facilitate smooth transitions from hospital to home, rehabilitation, or long-term care facilities.
Educate patients and families on post-discharge care, medications, and follow-up appointments.
Monitor and track patient outcomes to ensure quality care and reduce readmissions.
Maintain accurate documentation and comply with hospital policies, legal, and regulatory requirements.
Advocate for patients and families to ensure access to appropriate healthcare services.
Qualifications:
Valid RN license.
Bachelor’s degree in Nursing (BSN) preferred.
Experience in case management, discharge planning, or related field preferred.
Strong communication, organizational, and problem-solving skills.
Ability to work collaboratively with multidisciplinary healthcare teams.
