Job Description

Job Title: Registered Nurse (RN) Long Term Care (LTC)

Job Summary:

The Registered Nurse (RN) in Long Term Care (LTC) is responsible for providing comprehensive nursing care to residents in long-term care facilities. The RN will assess patients, create and implement care plans, administer medications, and collaborate with interdisciplinary teams to ensure the highest quality of care for residents.

Responsibilities and Duties:

1. Assess, plan, implement, and evaluate nursing care for residents in long-term care facilities

2. Administer medications and treatments as prescribed by healthcare providers

3. Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate care for residents

4. Monitor resident's vital signs and condition, and communicate any changes to the healthcare team

5. Educate residents and their families on health management and disease prevention

6. Ensure compliance with state and federal regulations related to long-term care facilities

7. Document all patient care activities and maintain accurate medical records

Qualifications and Skills:

1. Valid RN license in the state of practice

2. Minimum of 1-2 years of experience in long-term care or geriatric nursing

3. CPR certification

4. Strong communication and interpersonal skills

5. Ability to work effectively in a team environment

6. Excellent organizational and time-management skills

7. Knowledge of state and federal regulations governing long-term care facilities
