Position: Registered Nurse (RN) – Home Health
Department: Home Care / Community Health Nursing
Reports To: Clinical Nurse Manager / Director of Home Health

Overview:
The Registered Nurse (RN) – Home Health provides professional nursing care to patients in their homes under physician direction.
This role focuses on promoting patient independence, preventing complications, and ensuring continuity of care across healthcare settings.

🧩 Key Responsibilities:

• Perform initial and ongoing patient assessments in home environments.

• Develop individualized care plans based on physician orders.

• Administer medications, wound care, and other prescribed treatments.

• Educate patients and family members on health management and recovery.

• Coordinate with physicians, therapists, and social workers for comprehensive care.

• Maintain accurate documentation and update electronic health records (EHR).

• Monitor vital signs and report changes in condition promptly.

• Ensure compliance with agency policies, procedures, and regulatory standards.

🎓 Qualifications:

• Current Registered Nurse (RN) license in practicing state.

• Minimum 1 year of clinical experience (home health experience preferred).

• Strong communication, time management, and organizational skills.

• Proficiency with electronic documentation systems.

• Valid driver’s license and reliable transportation.
