Job Title: Nurse Practitioner – Geriatric Medicine

Job Summary

We are seeking a compassionate and experienced Nurse Practitioner (NP) specializing in Geriatric Medicine to provide high-quality, patient-centered care to older adults. The NP will assess, diagnose, treat, and manage acute and chronic conditions while promoting healthy aging, independence, and quality of life.

Key Responsibilities

Provide comprehensive primary and specialty care to geriatric patients
Perform physical exams, assessments, and diagnostic evaluations
Develop and manage individualized treatment plans
Prescribe medications and therapies as per scope of practice
Manage chronic conditions such as diabetes, hypertension, dementia, and arthritis
Conduct preventive care, wellness visits, and health screenings
Educate patients and families on disease management and lifestyle changes
Collaborate with physicians, nurses, and interdisciplinary care teams
Maintain accurate documentation in the Electronic Health Record (EHR)
Ensure compliance with healthcare regulations and safety standards

Required Qualifications

Master’s or Doctorate in Nursing (NP Program)
Active Nurse Practitioner license
Board certification (AANP/ANCC)
Experience in Geriatric, Primary Care, or Long-Term Care
Strong clinical, communication, and patient-care skills

Preferred Qualifications

Geriatric Nurse Practitioner (GNP) certification
Experience in assisted living, nursing homes, or senior care facilities
Knowledge of Medicare/Medicaid guidelines
